- If a Beneficiary wants the Original Death Certificate returned, please do not send the original to the above address. Instead, make a high-resolution copy and send the copy in place of the original. Please keep the original in your office until the claim check has been mailed.

- Please Note: Submission of Yearly Audit Reports is Required by All District Councils as outlined in Sec. 131(c) of the General Constitution, and by all Local Unions that meet the criteria as outlined in Sec. 179(f) of the General Constitution.
